For FQHC and RHC clinics, HCPCS code G0511 replaces the 9945x series of codes when submitting claims using the UB-04 form. Starting in 2025, up to four units of G0511 may be billed in the first month, with up to three units allowable in subsequent months.
For Medicaid claims, reimbursement policies for Remote Patient Monitoring vary by state. Providers should contact their local Medicaid office to confirm whether 9945x codes or 99091 is applicable and to inquire about reimbursement rates
